I think that only players who've maxed out resets in the Uchiha Ichizoku and the Rinnegan should claim Perfect Susanoo, but hey, that's just my in my opinion.

Besides enhancing Madara's vitality and granting him usage of Wood Release, I don't think Mokuton has any other real effect(s). Of course, future chapters may prove otherwise.

I don't think Rinnegan plays a part with Susano'o. Did Madara not obtain the First's DNA after their last encounter? I also believe that Mokuton is needed to claim Sharingan evolution to Rinnegan. Just a heads up. So if that was the case, and Hishirama took him on with perfect Susano'o, then Rinnegan played no part in Susano'o's completion.

I mean he could've just Amaterasu'd the Mizukage, Hokage, or Tsuchikage. He could've easily gotten all of them when he K.O.ed them with the flower pollen.

I actually don't think it's either.  Kishimoto was probably just trying to milk the fight between Madara and the Kages for all it's worth.  If Madara KO'ed all of them using Amaterasu within a chapter and a half, that wouldn't be fun for the readers.  After all, he's beginning to wrap up the Naruto series from the looks of it.  I don't think it has anything to do with helping the kage grow nor just playing around with them.

And I agree with Solo Iori.  It's suggested that Mokuton is required to evolve the Sharingan since Madara was only able to unlock it once he had gained some of Hashirama's Moku DNA after their battle in the Valley of the End.  It makes sense.

But all in all...these characters are getting almost as ridiculously OP as characters in Bleach D:
