Well you are either issued a Deathmatch or both parties are aware before hand that character death is imminent. Otherwise as I stated before, Shinobi can battle without having to kill each other. There is a respect for mercy in letting one learn from their loss and mistakes in hopes that they'd get better.

Like with Hanzo. He let several people go who become tremendously stronger as time went on. You can be powerful and still have honor, though even with honor you must know when to put the enemy down.

It is certainly a part of the job description. Even genin Naruto croaked him a frog or two before the timeskip.
