It's a messed up world in Naruto... Besides, smoking kills you, it doesn't give you the rewards an apple would.
Marriage, doing stuff ten-ten, and alcohol can kill you (tenten is the devil

K... and that's probably why Sichae's shop doesn't sell those things.
